Bug#493549: Please remove duplicate "MENU TITLE" call

Hi,

Please remove the first call to "MENU TITLE" in pxelinux.HEAD - unless I'm
missing something, it never gets shown as it gets overridden by the second
call.

I've attached a patch attached to make this clearer. If I am missing
something, please comment the duplicate explaining its functionality. :)
