No, the connection is not loose.
It looks like that this issue only happens on "Apple" earphones (as well as the 
"Apple mode" on some earphones). There is a button on the earphone, and if I 
hold down the button, there is no problem playing. But I cannot hold it forever.
The distortion problem is Linux-exclusive. I do not know the course. All music 
files have this problem but not very noticable. It becomes noticable with 
speech. Holding the button solves the problem.
The "wrong channel" problem exists in Windows too, but not in devices such as 
an Android phone or a Nintendo 3DS. Again, holding the button down solves the 
problem.
Maybe it is related to this:

Apple has its own implementation of TRRS and I guess the button disables the 
mic and other "special features".
So it is a hardware problem after all. But I hope you to provide some sort of 
software workaround.

No, I am not talking about clipping. The problem happens regardless of volume.
If I run "speaker-test -c2", both speakers have sound output at the same time 
(which should not happen).
The "constantly swapping" does not always happen-it happens at random, and only 
on some of my audio files. I do not know what caused it.
Some observations when I adjust the volume in "alsamixer" and run "speaker-test 
-c2" (it seems that the swapping problem does not happen):
L=0, R>0: Both have constant sound on "Front Left", Both have no sound on 
"Front Right". (No, this is not a typo)
L>0, R=0: Both have no sound on "Front Left", Both have constant sound on 
"Front Right".
L>0, R>0, L>R: Both have constant weak sound on "Front Left", Both have 
constant strong sound on "Front Right".

What happens if you decrease volume by alsamixer?

Are you sure? Distortion happens when the volume is increased to
more that 100% in i.e. pavucontrol.
