 I tried upgrading my broken system directly to the 2.86.ds1-18
 version to try and fix my boot process.  Didn't work.  Had to take
 it down to 2.86.ds1-15 to fix it.

Can you provide more information, error messages, observed behaviour
or other things that help us debug it?  As it stands, this bug report
is unsolvable as there is nothing in it to work on.

Hi,

after fully recovering from the update-rc.d breakage, I also still have
trouble booting with sysvinit version 2.86.ds1-18. Downgrading to
2.86.ds1-15 solves the problem.

Upon trying to boot an LVM on root system, with a custom kernel and an
initrd image residing on a small boot partition, I receive an fsck
error message as follows below. I have attached dumps acquired from the
rescue shell of /proc/mdstat, /proc/mounts, my fstab as well as the
output of fdisk, lvdisplay, vgdisplay and mount in order to provide
some detail on my system.

I hope this will help resolve the issue in some way.
