I've discussed this issue with the PROJ.4 developers [1], and they've
pushed a fix for the upcoming 4.9.3 release [2].

The huge value determination in 4.9.2 changed that enabled the
transformation of points outside the datum shift grids, which are
incorrect for the RD/NAP to ETRS89 direction.

proj (4.9.2-2) was just uploaded to unstable and includes the fix for
this issue.
