It's poorly/inaccurately documented in all versions of Delphi I've 
ever experimented with. FWIW, the help file for TCompress 
(download via http://spis.co.nz/compress.htm) has instructions in 
the "installation" section for installing *itself* in with Delphi's help: 
 
a) Work reliably, as far as we've ever seen

b) Will probably work for yours too

Caveat: I don't bother with the .cnt files, but I believe they work 
pretty much as advertised in the Delphi docs.  Big hint: delete 
the hidden "delphi*.gid" file before restarting help. Otherwise it 
never has cause to rebuild it and hence never "sees" your help 
file...

NB: You gotta have the right "keyword footnotes" -- type "A" for 
D3/D4 I believe, type "B" for earlier versions of Delphi, IIRC.

Thanks for the help[1] So far I've managed to get the context sensitive help
working, but my topics don't show up in either the table of contents, the
index, or the find dialog.  mind you, half of the delphi topics don't either
:)
