On Karmic, I'm seeing buttons that have an underscore instead of an
underline for the accelerator key. It's more obvious what I'm talking
about by looking at the screenshot I posted.

Umm we're absolutely sure this is a bug in each individual package,
right?
Something must have changed in gtk that caused use-underline to be false
by default. I don't know what would have caused all these packages to
suddenly have problems.

I am not agree with you. I cannot do a dist-upgrade without pulseaudio
pacakge to be installed in addition. The only way to do it now is to
fixed the gnome-settings-daemon. I don't know exactly what is inside it
but perhaps that will be a good idea to separate the different component
and put
still the same problem. Dist-upgrade try to force me to install
pulseaudio not agree with the answer.
