I'm having a very similar issue. With 21.10
I noticed that the issue goes away, when I change the scale to 100%
(instead of 200%) on the secondary screen.
Main Screen (USB-C): 3440x1440, 100%
Secondary Screen (Built-In): 3840x2160: 200%
I'm having the issue with or without extensions.

A few changes for gspawn landed in the 2.67 series that had some bugs
which were later fixed and backported to 2.70 as seen in
https://gitlab.gnome.org/GNOME/glib/-/merge_requests/2394. These bugs
are present in 2.68 as on impish, but that series won't see any more
updates
