I think this is fixed in Ubuntu 13.10. For me both screens are set to
their native resolution at the login screen and in the guest session.

Actually I just realised thet it might have nothing to do with totem, it
is a general display problem. When playing a game in fullscreen mode and
trying to change sound level the same flash can be seen.
