This was a PEBKAC issue. I was opening the Unity sharing control panel
instead of the GNOME sharing control panel.
I have no idea why the Unity sharing control panel is still on my computer. I
thought unity was gone. ;-)
Is it safe to remove it?

Desktop performance was good on 23.04, I'm currently trying out 23.10
and have found the cursor feels like it's running at a refresh rate
lower than the monitor (feels about 30fps, monitor is 60fps). Animation
when exiting the overview don't appear to complete properly often
No errors, my pictures are in 'Pictures', I have no problem for adding a
picture from 'Pictures' but ater adding a picture Settings - Appearance
- Add Picture returns to 'Recent'

Thank you for your bug report? Could you do 'journalctl -f' and see if any
error is printed while selecting the folder?
Where are the images stored? on a local drive directory?
it seems the portal interface is used to access the content so could be a
problem with the portal

Can change to select from Pictures or other folders but adding a new picture
returns to 'Recent'
instead selecting Pictures as before.

This sounds like either a Settings bug, or just a natural consequence of
(accidentally?) removing some dependencies from the default install. The
latter I think is also the responsibility of the Settings app to resolve
or communicate to the user what's missing.
