Annotations in dual mode only sees left page

When trying to add annotations to a document, when viewed in dual mode,
it is only possible to annotate the left page. Nothing happens when
trying to annotate the right page.

Saving a document after adding annotations, causes the text in
previously saved annotations to be lost. The annotation icon is still
visible in the document, but the text field is empty.
