I'm experiencing this as well, after an upgrade. rhythmbox doesn't play
the first time I hit the play button or double-click a file. The play
button is toggled, but playback doesn't start. If I click it twice or
double-click the file again it works fine.
Everytime playback is stopped completely
I had this problem once when the lookback interface wasn't up for some reason.
Just in case it's your problem here, here is a quick list of things to check:

I can't seem to reproduce this. However I'm still having this bug when
using the standard theme, see bug 34547 (which contains the link to
the trivial patch that was applied upstream to fix this)

FWIW, mlabel from the mtools package /should/ be able to adjust vfat
labels. For other filesystems, you might want to have a look at
xfs_admin, reiserfstune and e2label.
