OK the Totem developers at GNOME Bugzilla say this is a packaging
problem and that "Some libraries that the gst-libav package relies on
are missing." If I try "apt-cache search gst-libav" I see this is
"gstreamer1.0-libav", which is already installed.
I have followed their advice and filed a new
i dont know if it helps, but if i recall correctly in unity was a similar bug
and it was resolved.
Now in unity you see the secuence for any app of libreoffice
1.- Click in application
2.- In the dock, opens the white icon
3.- When app start, the dock show the correct icon.

I have installed a PCI soundcard & it demonstrates same behavior. With
amplification off, volume at 100%.
Turning sound down seems to cure, but obviously this is not loud
enough!?!

Had the same issue on 14.04 with HWE kernel.
Last Friday morning I got offered the -116 kernel - which broke the
nvidia driver module, forcing me to revert back to the -112 kernel.
Only today (not checked during the weekend) I got offered the compiler
update, from 4.8.4-2ubuntu1~14.04.3 to

Thanks for responding. Every single one of those packages are already
installed. If I drag a video into an empty Firefox tab, Firefox will
play it. Same goes for Chromium. The files are definitely playable.
If I open the same file in Totem, it will not play. This is a new
