FWIW, I've tested mainline kernel 4.2rc3 and wasn't able to get to the
point of reproducing the issue because it wouldn't even show the mouse
pointer on my Dell XPS 13 9343.

I've also seen on my Fujitsu U904 (3200x1800) connected over DP to the
external 4k screen. I've worked around it by using Ctrl to highlight
the cursor position until the cursor returns (sometimes it takes more
than a couple of hours, and the only sure way to get it back is to log
out and log back

FWIW, I used ubuntu-bug -s sound to collect data and it put the bug
into pulseaudio, though I believe it's more likely to be a linux bug.
I'll leave that to triagers who know better than I do.
