I have the same problem. Restarting pulseaudio or reload of alsa (as
suggested) does not help. Restarting make it works again, but only for a
short while until it stops (again). I've tested with the stock 13.04
x64 kernel, and 3.9. No difference. Audio worked fine on the 12.10
system I upgraded
Restarting THE SYSTEM make it work again (rebooting) make it work
again I should have written.

Same issue on a Thinkpad W530 (with Nvidia Optimus + Intel HD4000). Uses
Intel driver by default. Second screen is not detected. On Quantal 12.10
x64. Also tested xorg-edgers (with no success and messy graphics as
mentioned in this thread) and ubuntu-x-swap/x-updates (which is
currently off-line
I do not think it has been fixed; I can't know for sure, but even my
desktop seems to suspend when I leave it alone for enough time. And
I've checked the power settings; looks fine (set to don't suspend
during install it seems).
