I noticed that this happened when I have the option "Switch between
minimized windows" off from the Unity Tweak Tool --> Switcher Tab. It
seems to not happen now that I turned that on again.

I experienced this problem mostly on Chrome, but I guess it would happen
for other windows also.

The reason I had the option disabled is that it was a way to make the
"show desktop" hotcorner work. It doesn't work now but I prefer this
from missing the cursor.

Bug description:
  After I minimize/maximize an application the handling of local window-
  coordinate sometimes will be broken.  The actual position of the mouse
  pointer in the window is lower the the position of the cursor on the
  screen (about the window title height or unity panel height). Most of
  time it happens with firefox (after fullscreen flash-video)  but also
  with other apps.

  I have same problem in previous versions of Ubuntu, if the "window
  placement" plugin was disabled. In this case the plugin is active.
