Saving a filled-in version of the file attached to comment #4 of the
upstream bug report (an IRS tax form) works here with the following
setup:
Ubuntu 15.04 "Vivid Vervet", Evince 3.14.2-0ubuntu2.
Both the default file name and an edited file name save correctly.
Reopening the saved file shows

Thank you for taking the time to report this bug and helping to make
Ubuntu better. The issue you are reporting is an upstream one and it
would be nice if somebody having it could send the bug to the developers
of the software by following the instructions at
I believe it only occurs when I edit the file name before saving.
Default file name seems to save okay though further investigation may
be necessary.
