Still happens for me in 12.10 when the network dialog pops up for example. (See screenshot)

Totem DOES temporarily exit fullscreen (for 50... maybe 100 milliseconds) and then pop back to fullscreen again. You have to <Alt><F4> or xkill to close it. Once totem has entered this condition it remains that way regardless of playlist... starting another file from nautilus (if you have a dual monitor setup or you <alt><tab>) no errors are printed to stout except for: "sh: jackd: not found" which is printed to stout on startup... and when each video file is started hope this helps you sold a VERY annoying bug... good luck!
