Note that the last test with 2.21.7 was with kernel 3.8, so actually not
as conclusive as I hoped. Any chance you can try testing with v3.10 as
well to rule out a few bugs there? And shortly there will be 2.21.9...

That one monitor loses sync is a kernel modesetting bug. Note that
apport failed to upload the dmesg so I can't check for any likely causes
of the freezes. However, if you can grab a drm.debug=6 dmesg for the
both classes of failures, I may be able to resolve them.

Here's the drm.debug=6 dmesg output for the xorg.conf configuration I've
pasted above (UXA).
I can confirm that this is a bug in xserver-xorg-video-
intel_2.21.5-0ubuntu1. I downgraded to xserver-xorg-video-
intel_2.20.9-0ubuntu2 and the issues went away.
