The description of the problem is quite similar to the description at
bug 1162836, where the culprit proved to be the likewise software for
communicating with other platforms. Are you possibly using likewise?

I am not using likewise. There was indeed something weird with our
(supposedly paranoid) pam.d configuration.
common-account:
account sufficientpam_krb5.so
account sufficientpam_unix.so
account required pam_deny.so
common-auth:
authsufficient pam_unix.so
and the issue disappears. At first glance it would seem both only
require pam_unix.so to succeed?
The issue was the use of 'sufficient', which terminates the PAM stack
immediately and doesn't process any modules listed further on. The
recent change relies on pam_env being executed after other
