This affects both my USB audio devices. The new device shows up in Configuration panel, but is not available in the Output panel until I "kill" pulseaudio and let it restart itself. It will work correctly on subsequent re-insertions.
Incidently, another minor annoyance - if Rhythmbox is playing, its title + the "simultaneous output" driver's long name stops the window from being resized.
