Bug description:
  Every time I log in on Ubuntu, it seems the volume is set back to 100%
  automatically. Normally, this wouldn't be a problem, but it seems that
  since upgrading to 11.10, any setting above the "Unamplified" mark on
  the slider, results in distortion. In 11.04 this was not a problem.

Ubuntu 11.10 (oneiric) reached end-of-life on May 9, 2013.
And Ubuntu 12.04 (precise) reached end-of-life on April 28, 2017.

Hi,

This bug affects me too on Ubuntu 12.04.2. I can't find the reason and I
just added this line to /etc/rc.local to make it mute when boot to the
LightDM:

amixer set Master mute
