the bug reporting skeleton has become so useful for me (and others) that I
have created two new skeletons:

http://perl.apache.org/~geoff/Apache-Test-skeleton-mp2.tar.gz
these are essentially the same as the bug

No. It borrows some ideas from various other modules, including
mod_cache and mod_file_cache.

When using mod_jk2 and mod_charset_lite mod_charset_lite cores because
r-filename is NULL.
The easy patch is attached.

patch looks reasonable... unfortunately, I lifted some bad style from
somewhere (the code around your patch)... I'll fix that up and then commit the
appropriate patch
