At least on my system (10.6.3) I had to add:
CPPFLAGS = $(CFLAGS)
... to get the Makefile to build build_geometry.cpp - otherwise there were no
CFLAGS set, and the build failed.
It might be worth mentioning on the wiki that you need to do a 'sudo port
install postgis' as well - ok,
