We should disable usage of the default calcite enumerable convention when 
running queries so that we
- recognize in our tests more easily where missing Phoenix implementations exist
- don't compile something down that won't scale well (though functionally it 
may work)

Once the Calcite Phoenix integration is complete, we can potentially go back 
and allow the default calcite enumerable convention on a case-by-case basis.
