I have updated the wiki page containing the CA incident report template.

The purpose of this update is to place more emphasis on the level of detail
that CAs should provide, especially in sections 6 (root cause) and 7
(remediation) of incident reports. Other changes were made to ensure that
all types of incidents (i.e. not just misissuance) can be adequately
explained.
