 a co-author?

It's generally accepted practice to put put your name on the copyright
even if you based the code on another driver since you were the one to
put it together and get it working for your hardware.

something like that.  The attribution should be a separate entry (e.g.,
line or paragraph) to avoid confusing it with the original one.
