Unfortunately custom forms are not properly handled in dataset reports. A work
around will be to “print" the dataset report output to pdf using Chrome.

That functionality had a number of issues in 2.22 and has been removed
completely in 2.23 and later versions. I will advise you use the dataset
assignment editor app if you are only updating a few assignments.
