If you are using Android App please follow Elmarie's suggestion but if web
tracker capture then it must be another problem.
Which version are using using, we also experienced a similar problem with
assigning users in 2.28 when you assign and save then come back to edit,

individual Orgunits that is also assigned to the programme and not higher
