Odd. I've seen this happen with one file before, but I don't know if
iTunes did it or SC. And it was ages ago.
In my case it was because I had a colossal number of characters in the
folder path and filename.

I have longer filenames and paths, but that does look a likely cause.
Though I can't understand why it hasn't happened before.
iTunes is running, but the time of creation is exactly the time of the
Squeezecentre scan, so I'm pretty sure iTunes isn't doing it.
