That's old code, and pre-dates detailed git logs, but it's fairly clear
that the test is implementing this part of RFC 5107
When servicing a DHCPREQUEST message, the DHCP server would normally
look at the Server Identifier option for verification that the
address specified there is one
Do you have an interface configured with an address in fd01::/64 ?

Sounds like you're just tickling the limit. Maybe just increase it with
--dns-forward-max
