Yes, I would like a title attribute on the a element. I can specify a
title for xlink:title, but the XML is then not valid. I can, however, still
transform it to XHTML, but the title does not appear within the a element
in the XHTML code. I can see that in the stylesheet inline.xsl,

The options with pure XML are too limited with set. The templates that process set
handle only the enumerated elements set, book, and setindex, as well as the automated
TOC. That gives few opportunities to insert any other content, even if it were
invalid.
The closest I could come was

release of the stylesheets, as a title attribute is important from an
accessibility point of view.
