stylesheets), and I've noticed that with the upgrade, the space-before
attributes in my chapter.titlepage.recto.style attribute-set override no
longer do anything. The space-after ones still seem to be turned on and work
fine.
All fixed. Turns out I had a template override for the thead template that was
missing the start-indent=0pt attribute in the fo: table-header.

The FO that Saxon produces looks the same between good and bad examples,
and I can't determine a consistent algorithm explaining when FOP 0.20.5
