Probably because the TOC does its own parse of the tree, so the exclusion
logic would have to be applied also.

I get a duplicate in the HTML output resulting from compiling
a DocBook article, using "Modular DocBook HTML Stylesheet Version
1.70" (DSSSL) with Jade version "1.2.1" SP version "1.3.4", and with
"-V nochunks" in the command line to disable chunking.

I am not sure but I think Michal Kay did some major performance
improvements. It seems that 6.4 is much faster.
