Several times over the last week I have noticed that the when I start firefox,
the default icon is replaced by a grey square with a question mark in the
center as shown in the attached image.
The only fix I've found is to unlock firefox from the launcher, quit the
program,

When I use ctrl+alt+[numpad 5] to maximize the currently active window,
the window does not move the window title bar into the global title bar.
I've attached a screenshot to demonstrate.

indicator and scroling, or by clicking on the indicator and dragging the
widget - the volume will jump up and down rather than smoothly
