ALT A. QRG and QSX are already there in the appropriate boxes read from the
radio but QSX appears duplicated in remarks when I send it.

If you are the 'spotter' and using Ctrl A to spot the DX, put the QSX freq
in the QSX box and it won't show up in the remarks.
Otherwise DX4WIN just feeds the remarks thru from the DXcluster you are
using.
