Has anyone used a software package which can use scanned questionnaires and
create a database for analysis? Has the results been satisfactory?

We are doing a survey of diabetics and their medicines and herbal remedies.
We hope for hundreds of responses and would like to automate data entry to
the extent possible. We have no software for that but our modest grant
could pay for same. 

The questionnaire could be created on printer or go to a print shop. The
first batch will be just LaserJet printer output, copied on copier and then
results hand entered.
