Revert to computing the dependencies via explicit Python code in `setup.py`
Using PEP-508's environment markers for conditional dependencies was a
total fiasco; see more extensive comment in the sources.

Bump `setuptools` requirement to 21.0.0
We cannot just pick a version number from the `setuptools` changelog: if
the code was not effectively released on PyPI, we are going to hit a
"zipfile.BadZipfile: File is not a zip file" error as per
<https://github.com/pypa/setuptools/issues/579>
