Interesting reading. I sure second K2TA's comments.
Here are some of my experiences and wisdom with qrp.
The question, Who does the work? demonstrates what I see as a major
issue with most qrpers; they expect to be weak. They also assume
that the other guy will have a hard time

I have experience the main component of what you have described,
namely, the high S meter reading with the preamp on.
I have intermittent line noise here (luckily, mostly gone). When the
noise is off, I can switch in the preamp and there is no change in
the ambient reading on the meter.
For those of you with no interest in QRP contesting, delete now.
When I'm qrp, I frequently (almost incessantly) sweep the band over
and over and over looking for new contacts. My method is to start at
the bottom (say, 14000) and tune up towards the top. At some point, I
decide it's time to go
It will show the offset when you actually transmit.
You should engage both the RIT and XIT and then when you wiggle the
offset knob, you can see the qrg change. Then release the RIT and
you're back on the correct listening qrg and ready to xmit on the qrg
with the desired offset.
It is a pain.
