Aloha Mike,
Inconsolata is a pretty monospace font, but it appears it isn't widely
distributed yet. Another drawback is that it doesn't have a bold face
or an italics face. Many syntax highlighting schemes use these faces
to distinguish comments, reserved words, etc., so Inconsolata is
below:

Looks like something went wrong so I reinstalled MikTex 2.8 and tried again. I
started off with simple PDFs and slowly built up.
I am finally able to get a pdf version that is close to yours.

I don't know what is causing that. I've never seen it before.
Are you able to compile successfully with the times option (which
doesn't use the inconsolata font?)

Good news.
I'm not sure what you mean by close, though. In the LaTeX world, a
document is supposed produce the same output regardless of the
platform or the output device. Close is typically not good enough.
