Hi Pravesh,
Copy and paste the below formula in C15 cell
=VLOOKUP($D$2,INDIRECT('$D$3'!$A$1:$E$7),MATCH(C$14,INDIRECT('$D$3'!$A$1:$E$1),0),0)
Drag the C15 cell formula to Right upto F15 cell.
