We had a similar problem - traced it to the offline folders, just delete those and re-create and the problem went away.
For us it started after we moved people from one server to another. I thought it was only happening to Office XP users, but know it is happening regularly to about 30 out of 500 mailboxes. About every 10 minutes (even though logged into the domain, about 20 on the LAN, 10 through dial-up) the Exchange server pops up the username/password/domain authentication box. No matter what you enter your password is wrong. Happened to me the first time right after I changed my password so I thought it was a domain sync issue. Doing that didnt help. You can hit cancel and go about your merry way without being denied access, but it is a pain in the @ss . . . Any ideas?
