Hi, original reporter here.

This is FreeBSD (61=ECONNREFUSED) and no hosts_max_try* options are configured.
Exim patched with
https://git.exim.org/exim.git/commitdiff/06f37c802cbe477b2db358a0e76388d2df84691f
does not suggest that hosts_max_try* is a reason for this behavior.

Alternatively, set delay_warning_condition to only send warnings if the
email was sent by one of your own users.
