Two possibilities are: a clot (but if it's gone on for awhile, perhaps not) - 
has the kitty be screened for any heart issues, like HCM? they always tell me 
to watch for trouble with the back legs with this disease. 
 
Also, I have a kitty who had his legs go out on him and he'd be dragging them 
behind him - this was when I first got him.  I took him to a holistic vet who 
did chiropractic adjustments on him and it never happened again.  She said he 
had a misaligned vertabrae.  I did about 6 treatments and it cured him of the 
problem.  If you cannot find a explanation with regular medicine, consider 
looking for a vet that does animal chiropractic.
