I think it's intended to be thread safe. However, a couple of bugs (in 0.20.2) break 
this.

A (possibly not exhaustive) list of things to fix:

synchronizsation should be class-wide rather than object-wide in the following classes:

org.apache.fop.fo.StandardElementMapping
org.apache.fop.extensions.ExtensionElementMapping
org.apache.fop.svg.SVGElementMapping
