In that case, fo:table-cell display-align=center
is the only other option I can think of for centering vertically. I don't think it should be a problem if your external-graphic is placed within a fo:block and then within the fo:table-cell tag.

Try defining it in the fo:page-sequence. Your code within the fo:root should look something like below..
