process is to create PDFs by using the Print To File option, then

Thank you--your answer was spot-on! All I had to do was select each
chapter's text box containing the chapter heading, right-click, and choose
Send to Back. Now my TOC is happy and so am I.

It would be helpful if you'd provide more system info: your OS(s), what
version(s) of Adobe Distiller (full version of Acrobat, the limited version
that ships with Frame; something else, and versions), RAM, verify that temp
free space is adequate, and so on.
