Thanks muchly. I did have cross reference also in the paragraph element, but
it was like this:
TEXT | ( Emphasis | InlineGraphic | Button | CrossReference)*
I changed it to ( TEXT | Emphasis | InlineGraphic | Button |
CrossReference)* and this seems to work.
