Ok, this has gotten way weirder, but I have managed a partial solution
that lets me create this particular file the way I want. I suspect the
book will bomb out, though. I've always had trouble getting Save as PDF
to work with books. Here is what fixed it enough for now:

I checked using a simple rectangle and some text.
I checked the behaviour with FM 8 and FM 2017 (no FM10 installed)
Changing the color definition to "overprint" worked, as the pdf has
marked the black color as "overprint".

Whoa, I hadn't noticed the versions you were using. For all I know, it's
been fixed since FM 10. FM got an entirely new PDF engine in FM 2019.

ridiculous! They should work on composite PDFs, too!

Well, hmmf. Mine says "Page has Overprint: No," and knocks out
everything below it. I even tried using a drawn rectangle on the body
page, as you did, instead of a background graphic. Same result. I also
tried setting the color definition of regular black to overprint. Same

processes that involve multiple passes (AKA the old way.) Have you found
issues with misregistration on printers you control, or your client's
