The ruling line is most likely now behind the expanded header text
frame.
You can grab it by dragging a box around it - be sure you start outside
the header text frame and do not completely enclose the header frame
with the selection rectangle.
Or, you can select the header frame and send it

How/why would a graphic imported by reference not show up in the LOR?
I've found at least 4 graphics in two of my books that aren't listed in
the generated LOR (All are imported by reference.)
I'm using Frame 7.2 unstructured on Windows XP Pro 2002 SP-2, if that
matters.

A quick test shows me that the name of the referenced inset is visible
as plain text within the binary FM file (FM7.2). I can't say for sure
that this is 100% reliable, but I'll bet that it is. You should be able
to do a text search (using a reliable tool .. not the default Windows

If you want the FM integration with RH, you have to buy the suite. You
don't get it without the suite. So, there's that angle to consider as

designer Advanced tab, in the Frame Below drop-down. That setting
grabs a frame containing the blue line from your reference page.
To add another line of text to the header, create a new second header
