Yes, I searched and found this error had existed in earlier versions. This is 
the first time I have seen it and I have been using Frame for 20 years.I tried 
a couple of the hints that were posted, first removing the TOC and Index and 
updating the book. Same error.Rebuilt the TOC and Index, still getting the 
error.It looks like Jeff's response is spot on. An old bug has resurfaced.I 
can't find anything wrong with the book. All cross-references work fine. TOC 
and Index look fine.
Time for Frame to let the test guys loose, after checking what the fix was 
before.

Disconnected pages are supposed to work like that, IIRC, Ken. To add pages
as text frames are filled, FM needs to know that the text frames are in
connected flows. With Display Text Symbols ON, you should see a black line
at the bottom of overset text frames, unless the bottom of the frame
extends below the bottom of the page it's on.

Around 1995, I worked for a Silicon Valley Sun Computer and FrameMaker
reseller, who presented an "introducing Adobe Acrobat" sales event to its
A-list high-tech enterprise customers. Charles Gesche, one of the Adobe
founders, was the keynote speaker.

Yes, the Acrobat suite of products then was expensive, but our large
customers saw value in being able to automate the conversion of their
mountains of legacy documents, created in a variety of formats, into PDF,
and gain some order and control over their existing documentation chaos.

I asked our CEO if he thought that after the expected initial sales and the
rush to process all the legacy materials, if there would be much of a
continuing Acrobat market. "Once a customer invests in a large-capacity
software conversion system that doesn't wear out, why buy more?" He told me
to "ask Chuck." Chuck said, "I have no idea. It's early."

Over time, it seems that Adobe did figure it out

Thanks for the thoughts. I was pointed to the method for adding disconnected 
pages and then using a process to re-connect text flows and that seems to work. 
I cannot find any reason why some chapters just randomly stopped adding pages 
(at random lengths) but this approach does seem to work. I guess just more joys 
of using FM.

Um, not sure if this helps but I just ran a test here and if I add one 
disconnected page (in my case, right after the last page of the document I'm in 
right now) and then add text until the cursor is at the bottom and then keep on 
adding more text, no new pages are automatically added for that new text flow.
